Data controller

Data collected
Through the appointment request form: name, phone, e-mail, preferred specialty and practitioner, preferred day, time and language, and your optional message. Through the blood-test booking: name, phone, e-mail, date of birth (optional), the chosen time slot and your confirmation that you have a prescription. No data is collected for advertising.
Purpose and legal basis
Your data is used only to contact you to schedule an appointment. Processing is based on your consent (Art. 6(1)(a) GDPR) and, for any health information you choose to share, on your explicit consent (Art. 9(2)(a) GDPR).
Recipients
Requests are read by the centre’s front desk and, where needed, by the practitioner concerned. The website is hosted by Cloudflare, acting as a processor.
Retention
Requests are deleted once handled, at the latest after a period set by the centre (to be completed by the centre).
Cookies
The public site uses no advertising or tracking cookies. Anonymous visit statistics are collected without cookies and without personal data (page viewed, device type, approximate country and city; the IP address is not stored). Only the centre’s management area uses a technical login cookie. If you open a practitioner’s online diary, the Doctoranytime module is loaded from doctoranytime.be and follows its own privacy and cookie policy.
Your rights
You can request access to, correction or deletion of your data, withdraw your consent or object to processing by writing to contact@cmgmed.be. You can also lodge a complaint with the Belgian Data Protection Authority (Rue de la Presse 35, 1000 Brussels — dataprotectionauthority.be).
